How does the program work?

This program helps people with limited incomes who are enrolled in a Medicare Part D plan. It helps them pay for medicines that Eli Lilly makes.

You may be eligible for this program if you:

  • Are enrolled in a Medicare Part D plan, AND
  • Do not qualify for Medicaid, AND
  • Are taking an Eli Lilly medicine covered by this program, AND
  • You do not qualify for Medicare Part D's extra help, and you meet these income limits:
    • You are single and have an income of less than $20,800 per year. These income limits are higher if you live in Alaska or Hawaii.
    • You are married and have a joint income of less than $28,000 per year. These income limits are higher if you live in Alaska or Hawaii.
  • Or you do qualify for Medicare Part D's extra help and your income is:
    • More than $14,040 per year and less than $20,800 if you're single. These income limits are higher if you live in Alaska or Hawaii.
    • More than $18,900 and less than $28,000 if you're married. These income limits are higher if you live in Alaska or Hawaii.

If you qualify for this program, your medicines will be shipped to your home.

What medicines does it cover?

The program covers the following drugs:

  • FORTEO
  • ZYPREXA
  • Humatrope

How do I get in?

You must fill out and send in an application. With your application, you should send in:

  • A copy of your Medicare Part D plan ID card;
  • A prescription for the medicines you need
  • Proof of your income, such as a tax return; AND
  • A copy of the letter you received from Social Security saying you did not qualify for the Medicare Part D extra help and that:
    • You are single and your income is less than $14,040 per year — but these income limits are higher if you live in Alaska or Hawaii, OR
    • You're married and your income is less than $18,900 per year — but these income limits are higher if you live in Alaska or Hawaii.

If I get in, what are my costs?

There is no enrollment fee. If you qualify for the program, you will pay $25 for a 30-day supply of your Eli Lilly medication.
